Before patient discharge, or when patient condition change at home, our nurse coordinator will go to hospital/patient house to assess patient’s condition, speak to Doctor, hospital nurses, allied health staff and family members, then help patient to arrange  (buy or rent ) home environment which is suitable for patient to discharge to, including but not limited to:

Hospital beds

Medical equipment

Rehabilitation equipment

Medical supplies

Environment accessories
